Fort Haight, Mistwood region. Enter the fort and run up the stairs, then head to the left and up another set of stairs. These will take you to the top of the fort; head right and climb the ladder to reach the chest containing the left part of the Dectus Medallion. Fort Faroth, Dragonbarrow region, Caelid. Enter the fort (it's a good idea to have your Torch equipped to see where you're going, and have a spirit summoned to help out) and run past the enemies inside. Head over to the left-hand side of the fort and you'll see a latter. The medallion is in a chest at the top.

You can find the Grand Lift of Dectus above Raya Lucaria Academy in Liurnia of the Lakes.

Stand before the two statues until you see a prompt that asks you to present the completed Medallion. You'll trigger a cutscene and be taken to the Altus Plateau.
