Bloodborne - one of the best games on PS4, only one character you can't perma' kill I think but they're not technically alive to begin with.
Wasteland 2 - Anyone can be killed in this game and death is permanent. This includes any and all of the characters in your group. There are of course consequences for killing civillians or specific allies as you would expect as you're meant to be a sort of police force / service.
