How Do You Start New Game+ in Armored Core 6?

To start a New Game+ playthrough, you don't actually have to do anything at all other than beat every Mission. Upon finishing Chapter 5, which is indicated by the "Concludes Chapter" alert on the Sortie menu before you load in, you'll return to the Garage and the Missions tab will have automatically restarted the campaign. Skipping Illegal Entry, you'll be given Destroy Artillery Installations and Grid 135 Cleanup to choose from at the beginning of Chapter 1. You can now work your way through the game again and make different Decisions if you like to get a different ending.

What Carries Over into New Game+ in Armored Core 6?

The short answer is absolutely everything — literally. Upon beating the game, it automatically loads you into a New Game+ playthrough, but you don't lose any of your COAM, mech Parts, or access to previous Missions. You'll return to the Garage and have access to everything you did before the final Mission, just now you have the option of progressing through a New Game+ playthrough in the Sortie menu. You can still go back and replay every Mission you've completed, or return to customising your mech with new Parts. Absolutely everything is carried over; you don't lose anything.
