I'm not entirely sure what your question is, but you shouldn't be blowing out your eardrums. If the sound is distorted then either your headphones aren't very good, or they're broken. Try a non 3D audio setting. If it's the same then get new headphones.
@Abarth_71 switching it off didn't really make a difference, it seems like ps4 games the audio is loud i can turn down the volume, ps5 games seem to be quieter
