During the very beginning of the third mission in the campaign, titled Fractured Jaw, your controller may begin flashing and automatically disconnect itself. There are also reports of this happening during multiplayer matches and the Zombies mode. To stop this from happening, reset your PS4 by turning it off and on again. Then plug the USB charging cable provided with your PS4 into both the console and the controller.
Next, on the PS4 home screen, select Settings, then Devices, and finally Controllers. You'll then want to select the bottom option named Communication Method. Change the selection to Use USB Cable. If you do this, you must keep the USB charging cable plugged in to continue using the PS4 controller. You should now be able to continue playing Call of Duty: Black Ops Cold War.

I have had this problem as well im going to try and switching it from Bluetooth to USB
Lenny 0521:gamer ID: yes I just got done downloading the campaign and I am unable to play because I keep getting the flashing light on the controller.
Thanks this worked for me but the only problem was that my headset (wired) will connect and I can hear game audio and party chat audio but the mic won’t work. And if I switch back to Bluetooth the mic works again. But at least I’m able to play and I still talk to my friends through the PlayStation App on my phone.

Keeps happening to me
Changing to USB did not fix the issue for me. It still turns off my controller when I get to the Vietnam mission. I can't used the controller to fire the gun on the chopper.
