This "Behind The Scenes" trailer with the developers of Mafia II details just how hard. They make an excellent point when they state that Mafia II is not a "sci-fi game and therefore people have an understanding how things should work within the gameworld". And it's true — watching people wait for the traffic lights to change in order to cross the road is unlikely to be something you'd look out for when playing the game, but you'd notice it if the AI routines weren't programmed in. And that's an important part of making a real world. If you don't notice it, the team's done a good job.

We're getting kind of hyped for Mafia II. We find it hard to resist any gangster games, but this looks like an especially good one. We're stoked.
